Make sure you have these two email addresses setup from the domain your sending from: [email protected] and [email protected]
I have a legit double opt in mailing list myself and I found that doing this helped for some subscribers.

I'm using PhpList.
It's a standard inclusion with Fantastico if you have Cpanel inlcuded with your hosting.
If you don't have Fantastico you can also download and install it yourself from here: http://www.phplist.com/
I find it pretty good. Pretty easy to setup and you can customise your subscribe and unsubscribe pages.

The only problems I've had are with AOL. Sometimes the mail seems to get filtered, other time it gets through. I don't know whay that is.
Yahoo, Hotmail & Gmail (so far) seem to be problem free.
I've been using this now for 2 1/2 months and I'm pretty pleased.

I think you're missing the point. ;)

You have to get whitelisted for hotmail, aol, and yahoo which can be a process. I believe microsoft requires you to get bonded as well.

Either white listed, temporarilly beat their spam filters, or had the user manually add them to the not junk list. Go run some ex-memebers mailings and get delivery % numbers for msn\aol\yahoo and then rejoin the discussion.

I hate people who speculate based on no information what-so-ever.
