Photoshop with a couple sharpen / blur plugins is my guess... and pencil cutout or whatever that is to make the black lines.
Maybe the guy used multiple layers too ... |

From the looks of the sample you posted, it looks like to me whoever did it probably did something like what I linked to above, and then "cleaned up" some of the noise in the outline layers with the eraser tool and added in outlines where they weren't connected with a drop shadow brush (In the brushes pallete dropdown menu select drop shadow brushes). You kind of have to fuck around with it to get the look you want but I think this link plus a few of the ones posted above should get you on the right track. |
