He was a patient the whole time. The last 5 minutes reinforced it with the question he has his partner (doctor) and then the doc giving Ben Kingsly the nod to have the procedure performed.

The net is full of them, and what I am saying is that the whole story about him being insane is so easily bought. Anyone, even you, would come to the conclusion that he had in fact been hospitalized for X years, but if you read between the lines there are chances its not the case. In the book its clearly evident he was, but Scorcese had the chance to change the story to his liking and make the ending open for interpretation. The beauty of it all would of course be having 95% of the viewers thinking, "oh yes, haha, of course I understood the story" talking about "but what did he mean with that monster/good guy question in the end?" instead! Good film but not original, the idea of the person in charge actually being the inmate has been done. But I liked it, thought it was well made and its Scorcese...

Why is it so hard to believe there were indeed illegal experiments going on the Island and the docs were scared he was going to uncover them, so they kept fucking with him all the time he was on the Island (drugs) and in the end made him believe he was a patient?

The ending reinforces this when the guy knows there is no way out, but refuses to play by their rules and makes a conscious decision to get a lobotomy ("die like a man")

I predicted the ending during the opening scene... I was really hoping that the ending would be different and a little more complicated, because it was really too easy and simple. I thought Scorsese would have been able to come up with something a little less obvious

contrary to what fuzebox says, the average reading level in a typical newspaper i believe is like 6th grade... movies like this baffle the shit out of ppl... even me ;)
