He was a patient the whole time. The last 5 minutes reinforced it with the question he has his partner (doctor) and then the doc giving Ben Kingsly the nod to have the procedure performed.

Why is it so hard to believe there were indeed illegal experiments going on the Island and the docs were scared he was going to uncover them, so they kept fucking with him all the time he was on the Island (drugs) and in the end made him believe he was a patient?
The ending reinforces this when the guy knows there is no way out, but refuses to play by their rules and makes a conscious decision to get a lobotomy ("die like a man") |
i like the end were he wants to get fucked by doctors...
