Bone, once infected, presents little barrier to spread of infection to contiguous bone, and so it spreads within his skull. Ought to spend a good long time in the hospital, but he doesn't. The bone dies, and begins to erode. Weeks pass. At some point, the smell attracts flies, which begin to lay eggs in his festering wound, and maggots take hold. Weeks pass. The infection breaches the inner layer of his skull, and reaches the meninges. Weeks pass. Though their tensile strength is impressive, the meninges are quite thin, and the infection breaches them. Now, infection and maggots set to work on his brain. Your brain just isn't supposed to be on your outside, and presents almost no barrier to anything when exposed. Infection and maggots get to work on his brain. This makes him feel a little wobbly on his feet, and so, what do you know, he decides to see the doctor. He walks in to the Stanford ER, where these photos were taken, just as you see him here.

can you live with your brain outside the skull? did they push his brains back in and put some kind of steel plate and sew the skin together?

i was watching some educational/medical channel and they had some guy getting a hair transplant, not those plugs you see on all over, it was bizarre, they put some kind of balloon or something under the skin of this guy's skull and expand it, i mean REALLY expand it, your head looks monstrous, the top of your head is like a huge balloon, and the guy lives like this for months while his skin stretches, the effect is that the side parts of your head where you have hair stretches so it covers the top of your head.
End result looked ok but no better than hair plugs.

well, these images are going to stay with me for a good long while.

When you see what looks like maggots on a living body in a medical environment it is because they placed them there on purpose in order to eat the dead tissue.

This prevents gangrene and further infection. Apparently they just eat the dead tissue and leave the healthy stuff alone.

It's not because they let themselve rot so bad that flies planted their larvae in the wound. Although that would be bad ass.
