I'm checking some cam serps on google, i'm seeing a lot of spammy sites like abc-fettabsaugung.de, pblexposervice.de and others

Clicking on the link in the OP shows a pretty normal looking set of results. Not sure if it's a regional thing or the issue has been fixed.

You should search for "webcam sex" to see what appear on the first 3-4 pages... I don't understand how the big G algorithms are made these days...

Yes it is regional, that page got non-english words, .de domain, and shows only in some countries. Still it is a fail from google, sure iti s not an authority site with much of content. If you check results in every country these are often very different (note links below got safesearch=off, with safesearch enabled it even different):
