I have used CCBill, Epoch and Verotel. Epoch get my vote for the following reasons:
1) No hold backs.
2) Fast loading admin panel.
3) Good affiliate backend based on MPA3.
4) Awsome support.
5) Extremely fast and reliable payouts. (I somehow receive wires into my UK bank account the same day, where the others take 2 to 4 on average!)

I used Epoch for 6 years and then out the blue they terminated my account with no warning or discussing because of another website I was linking to from a Top-List, I removed the link immediately and they refused to re-instate my account, no support, no discussion and even my account manager would do nothing about it!
Awesome support? Erm not quite!!

Sounds like the toplist was yours if you could remove the link.
Was it your toplist? What was the url for the toplist ?

Sorry I probably didn?t explain that very well, yes it was my Top-List and a website who signed up to my top-list was linking to another website, which Epoch claimed was promoting potentially illegal material.
I removed my whole Top-List within 10mins of Epoch notifying me they was closing my account, so the Top-List URL is no longer active, but they still refused to listen or re-instate my account.
The real kick in the teeth is the website they claimed was promoting potentially illegal material had 4 banners on there of websites ALL using Epoch as there payment processor.

ccbill is a great stable processor that has redundant systems, amazing networking to make sure that transactions are always going through, and lots of dedicated staff to support it all...thats who my recommendation goes to

Segpay! They're sales and tech support is above and beyond any of those you listed. I do use almost all that you listed but keep SegPay as primary.

Plus Segpay doesn't hold or distribute funds, the bank does.

I work with Verotel, and I myself handle all of the merchant support, and personally make time for each of our merchants. We handle the end user support as well and have a team that works with the users in order to keep chargebacks and refunds at a low. I like Verotel. The new join forms are great and the admin panel is easy and fast to navigate.
However, I pulled you guys from the top of my cascade when I realised there is an extra 1.5% added onto rebills. Why is this? The Premiere account rates were very competitive until I saw this.
Then there is the tech support hours. Verotel do not have 24 hour tech support unlike Epoch and CCBill!

Just a couple of negatives there but I've always been paid on time with no issues.

You should also take a look at EPGBILL.

They have been around since 2005 and never had a late payment.

and still most affiliates won`t promote any other then nats or ccbill
