it usually "comes back" the same way it came in on the first place, last month I got some shit twice due to some Adobe whatever exploit. I did my Adobe whatever updates, and voila, never saw that shit again (knocking on wood)
so yes, after you clean house, try your best to find out how you got it so you can protect yourself
another vote for CCleaner though, run that too
PS The shit I got was really hardcore - blocking sites, multiple pop-ups, processes running in the background, changed the desktop background, disabled the Windows Task Manager, installed a fake boot up screensaver - that was really ingenious on their site since the moment you start running your Malwarebytes’ Anti-Malware you see Windows booting up and you start hitting the restart buttons and interrupt the scan, it is actually a screensaver, all you have to do is hit a few keys on the keyboard to get out of it
