For adult in the US, there are none that will allow you to bill direct to phone.

Outside of the US Bango.com is the easiest to setup.

Contact Marc or Allie over there.

You're probably already aware of them, but if not - have You checked out Password By Phone yet..?

They have plenty of options for You. Whether their solutions mean being paid enough for Your traffic or not is up to You to judge though.
Depends on what use You'll put it to.

These guys do phone billing primarily, not sure if they're offering SMS billing - my bad.. :\
But check 'em out at least, not impossible they do that too.
