Like lots of the Japanese bukkake videos, it's fake.
If you play it back frame-by-frame or slowly, you can see a pattern (hoses jetting out fake vomit from either side of the mouth, but not the middle). I can burp and make it sound like "beer" (after years of practice in college), but puking on command is a whole different barf game.
