I think it depends on the context. For example, if he wears a thong because he thinks it makes him cool, and girls (or guys) will be all over him, yes that makes him a fag. But, if he wears the thong purely for amusement or to get reactions from people, like to the beach, but acts like a jackass then it doesn't make him gay. Same with speedos.
