Are there any major web site development companies? Or do the majority of highly professional website get development by smaller scale operations or internally?

I'm interested in this too, I think there are mainstream companies that many not object to coding for an adult site there are also some companies like siteextreme.com

From what I have experienced, it appears to be more along the lines of individual contractors for the adult sector.

And more corps. doing things for corps (everyone needs to CYA and have a scapegoat it seems - opinions from living too long in a gov't town .

If anyone needs sites designed - I know a HIGHLY skilled artist who was just laid off due to those he was working for (a corp) went under. He has time and talent and won't cost a million.

It seems to me that every website is developed by small-time developers or by internal developers, in the case of huge companies, with huge marketing budgets...

Large sites are built by a different breed of website designer. A high end Advertising firm, IBM, and anything in-between employ them.
This is the usual team structure of a large corporate B to B or B to C site. It?s a pretty standard corporate dingo tree;
Project leader
Workflow team lead/manager
Site flow team lead/manager
UI team lead/developer
ASP/Java/C++ team lead/developer
SQL / Oracle team lead/developer
