We've looked a little into this with avs, we were kinda hoping we solved the issues visa had with our more closed door solution - let's hope visa agrees in the long term ;-)
If not, i think we are in a better position than most paysite owners to comply with whatever the new set of restrictions is ... we will probably have to look at a series of things like you mentioned over time ...

In theory ...
my concern is that people will spend more times in the sites with lots of text that are low bandwidth, and quickly browse through a movies section downloading 100 meg along the way in 5 minutes of surfing ... we haven't seen this happen yet, but i'm expecting there to be issues for some sites, yes.

Yes, I can so no reason why not ... as long as its an actual paysite standard members area and you can set up a copy with its own htaccess to post usernames to ...

About 650 at last count ... a week of beta testing and 24 hours since launch so we are expecting that to grow pretty quick from now on.

As much as that would be nice for the paysite owner, we doubt it will happen often enough to be an issue ... most of the paysites in the system are more expensive on their own LOL
Also, its in swoits best interest for that not to happen so we earn money too ....
