Unique clean quality web site design

Im about to start a new clean and non-adult video site. Im looking for a web site designer/programmer, who can make the site in html/css, but the site shoulf include a header.php and a footer.php. It should be easy to update links exchanges etc.

Here are some requirements:
-Should manage a 468x60 or 728x90 banner at top of the site, 300x250 banner "between" content or for ex. under/over the video player, and a 120x600 banner the higher right part of the site. These banners should on all pages.
-A rating system to the videos. Prefer some graphic rating instead of pure
numbers.
-A good visible and graphic menu with about 10 links to categories.
-A link partner area under the menu (atleast 40 links) and a link partner
area at the higher part of the site (about 10).
-A good CMS
-Different thumbs for the categories good places on the site which will send
traffic to my link partners.
-Absolutely no adult or questioable images, because it should be accepted in
Tribal Fusion, Calsale Media etc.
-Also a central place for a newsletter submitter
-Fast loading time, because of my ads. Still great looking graphic but not
unneccessary design.
