I have a celebrity site averaging about 20,000 uniques a month --Ive heard 1 in 500 should convert but Im not getting that more like 1-900
is this a really poor effort

A lot depends on the type of traffic you're getting, how much the site costs to join, what language the people speak etc. 1:900 isn't that bad of a ratio, depending on a few factors.

Many regions across the world can't pay using their credit card, either because they don't have any, or because credit card associations decline their payment

Usual billing companies just don't fit local needs and habits.
In case your traffic is not America-based, you need to check out Europe-based Billing companies
