Where the fuck does this error come from? Ive seen it a million times on diff web sites and it has recently plagued one of my php/mysql sites that was running fine for a long time. Im pretty sure it is not php or mysql related which leads me to believe it is a css or javascript error (which I have tons of on my site).

right at the very top of the page

Anyone know where this usually comes from?

include that is not pointing to the correct link or something. It's trying to call it but it can't because either the url or the file name is wrong. It happens to me whenever I don't put the correct URL in between that include. Follow?

can either be from a bad or missing include or permissions are set wrong on the file being written to

make sure you have the full server path. if not on dedicated email your hoster for the real one
