making model releases

i have recently started creating my own custom amateur videos for my clients, before i sell them i am curious if i am the owner of the videos ( i produce and star in them) do i still need to create a model release for myself?
if so, i have never made a model release, can anyone tell me which information must be on the model releases, perhaps show me a sample?

Depending on how you structure your company / business, you likely should have a release form on file.

EXAMPLE: If the content is real good you may get offers from other sites that want to use them / buy them / license them. If that is the case, you would need a release on file and it is best that the release be dated on the date the content was produced.

Furthermore, under the Federal law 18 USC 2257 you must make copies of your own ID and have it on file, and make many other records related to the content and where it is used.

Its actually a bit complicated. You are welcome to email me at [email protected] with any questions that come up or copies of a release you can modify for your own use.

MODEL RELEASE: ADULT

I, __________________________________
(Model), for good and valuable consideration, the receipt of which
is acknowledged, give to XXX Group ( represented byxxxxx ,Photographer), his legal representatives, successors, and all persons or corporations acting with his permission, unrestricted permission to copyright and/or use, and/or publish photographic portraits or pictures of me, and the negatives, transparencies, prints, or digital information pertaining to them, in still, single, multiple, moving or video format, or in which I may be included in whole or in part, or composite, or distorted in form, or reproductions thereof, in
color or otherwise, made through any media in his studio or elsewhere for art, or any other lawful purpose.

I hereby waive any right that I may have to inspect and
approve the finished product or copy that may be used in connection
with an image that the Photographer has taken of me, or the use to
which it may be applied.

I further release the Photographer, or others for whom he is
acting, from any claims for remuneration associated with any form
of damage, foreseen or unforseen, associated with the commercial or artistic use of these images.

I acknowledge that the photography session was conducted in a
completely proper and highly professional manner, and this release
was willingly signed at its termination. I certify that I am not
a minor, and am free and able to give such consent.

in addition to contracts i recommend all ID to be recorded on video station name and date of birth for fraudelent purposes.

In the case you're selling your videos to other parties, you also need to make sure that they have a license agreement with you.

So that they have proof that they are allowed to use your content.

(If we as a billing company, have doubts about webmasters having permission of using certain content, we ask for these agreements).
