I would suggest AVPStudio.com for what you require. They rock, are very resonable in price and the guy is not only a great designer he is an HTML god with a vast amount of sponsor experience doing members areas and everything else that goes along with it. I just posted the link to his AVPStudio page because that is the only one I have...but he does a hell of a lot of other things besides that.

A good designer knows how to:

1. Listen to the brief of his / her client
2. Put their own ideas and ego aside when they need to.

My clients were extremely happy with the finished results.
