Forms! Need help! Javascript!

I made this form script, but it dont seem to be working..could anyone help me with it..it's written in javascript.

It's supposed to send me the info you enter in the form??

As far as I can tell, there's no action="" in the form, which means the form data is not being sent anywhere. The FORM tag should look like this:

[form action="submit_form.cgi" method=post]

And then you should have a script (i.e. submit_form.cgi) that processes the form data and does something with it, like e-mail it to you, store it in a database, etc.
