Depends on the geo of your traffic.

I tried Exo, Juicy and Plugrush. In regards of cpm, I made with Plugrush nearly twice as much as with Juicy and a bit more than with Exo. I'd give each of them a try and check which works best as every webmaster will get different rates due to the geo mix.

Yep! Testing the way to go! It's not only the plane rate what decides what's best for you! It's also the conversion rate! If you get offered a monster comission, but you have no conversions, you still make nothing, so you should find a good balance between the commission and the conversion rate.
If you go for CPA of course.
