Sending a DMCA notice

OK, so this website stole my content. I have emailed the webmaster and asked politely for the content to be removed. No response...

So, I plan to send a DMCA notice. But, I never did it before. Any tips on how to go about doing it?

I have a standard DMCA form I've used twice, both times the pirated material was removed within a week.

I'm not sure if gfy permits me to copy/paste that form here, so send me an email at 'jesse at quinn69 dot com' and I'll send you the form along with instructions

It's pretty easy, you just need to make sure you complete the form and send it to the correct places (the site in question and their ISP host if the site doesn't respond to your DMCA)
