Looking for a PHP Form script a "idiot can even use"

He has all his forms (insurance quotes related ) pointing to a cgi script..

I need a "dummies" one he can use..


Anybody have a simple one they would recommend?

if you just need a basic form that sends the info to an email addy.
you build the form, link it to that script, and in the script itself, add you r email address and you're done.
