2257 and model release forms...

    2257 and model release forms...

    I was curious... can the 2257 and model release be wrapped into one document or do they need to be seperate forms?

    Any input?

      I have them as two different pieces of paper and scanned the same way. Separate.

              They often are in the same doc but its actually better if they are not.

              The 28 CFR 75 is silent to this issue though.
