Recommendations For a Designer/Light Coder?

I need a simple two page site.

page 1: Logo(will need it designed), some verbage and a "Click Here to Apply" link.

page 2: A form with some drop down lists, a couple "Browse" lines to upload some pics from their local drive, and then they hit submit and it comes to me all neat and clean.

I have to head out at this exact moment but if anyone has any recommendations on someone who can put this together successfully at a reasonable price, I'd appreciate the referral.

I'm still looking for someone who can do the whole thing for me and someone who takes paypal.

If I have to go with one designer and a separate coder, then I'll do that but I'd prefer not to.
