Anyone around to create a php form?

I need a simple email form created using php / sql or whatever you are used to using.

Basic fields of Name, email addresss, and text box, with a submit button to send the email. I have a design already setup with css file etc so you can match up colors etc to make it fit in nicely.

most cases i'm using wordpress, but not on this one. Got Bird working on it now.
