Need a script for code redeeming !

I need a script that does :

- customer/surfer visit the site
- they have a unique redeem code ( from a prepaid card )
- they enter in a page the redeem code
- it brings them to a new page where they can enter their email, select a - - user/pass
- email to them confirm u/p and duration of membership
- user/pass is written to password file ( or mySql db) with a duration of 30 days.
- original redeem code is erased , and cannot be used again
- u/p combination is erased after 30 days.

Using it for a prepaid card access.
