first time I buy from these guys, they sent me a pdf to fill, fax or scan, but I cannot find a way to fill it, I mean it is a pdf, just copy to a word processor and fill it there should do it. Looking forward comments from their customers.

If you dont have Adobe Reader, you can open it up in Photoshop - just add a white background, and your good to go with the text tool...

You can just print it out and then fax it off. Or scan it then email it back
