Payment processor

I need a good and stable payment processor that have the following:

CC processing, foreign and US
Less than 10% charged off every order
$100 max setup
No monthly fee
Ability to merge into a custom cart.

Any suggestions?

I use MVC but their fees are 10%, no setup fee.

I have them integrated into custom forms, I guess you could use them for a custom cart as well.

they charge the surfer whats fucked up imho. so your site says $9,95 and they add $1,50 on the join form
