Model Questionaire/application

Anyone out there have a good sample of a model application? No, not a fucking release. I was working on one of my own, but thought it might be good to see if someone has one I can play with. If you have one on MS Word 2000 lemme know. I am looking for something they can download off my site and fill out prior to the interview.

Let's assume I am just a purveyor of pornography and clearly state the nature of the photography will be adult-oriented. What sort of questions would you put on the application.

For instance, I have questions like "How did you hear about us?" and "What interests you about modeling for us?"

I don't use an application. I just talk to people, usually online. If you want to use an application, then all that matters is what *you* want to know. Why look online for a set of questions. If you don't know what the questions are, why ask?

Here, use these...

1. Why do you want to model for me?

2. Impress me with your experience.

3. Got photo ID?

4. Pick a color between one and ten.

Here is the method behind the whole application thing...

I want a girl to feel as though she is being treated as an employee. I don't want her to feel as though she is being photographed by some half-wit scumbag off the street. I want to know what she is going to do and what her limits are on paper that I can refer back to in five years when she is broke and remembers the solid check I gave her way back when. After interviewing N amount of girls a week I don't want to forget to ask anything in any interview. The application should cover those smaller but important questions.

Here's a few more questions I could think of:

"What is sexy to you?" - A broad question, but I could downright eat up the response and turn it into a photoshoot.

"What do you normally do to get yourself excited?" - Good to know whether or not I should throw the dildos in the dishwasher the night before or not.

Not the questions I would ask on a piece of paper. I am assuming that she is filling out the paper before you do the interview, so she really has no idea if you are a sleezebag or not.

I would go with the, name, age, address, height, weight, measurements, scars, stretchmarks etc etc, then give them options for various types of modelling - yes, no or maybe. Even if it is a no it gives you a reason to talk to them about it in the interview - then you can base how far you push it on the way she reacts.

We also just use a "please write a short description of yourself". The really dumb ones will write 'blonde (or any other hair colour), blue eyes, tall' - which has already been answered anyway. The ones worth shooting will tell you something about themselves, again another talking point to break the ice in the interview.
