- Federal Tax ID Number or Business License Number.....

Principal or Primary Contact Information
- Social Security Number
- Drivers License Number
- Passport



Now does the primary contact info HAVE to be in the US... or can i use myself in canada?

For Visa, you must be incorporated in the US.

Some banking institutes allow canadians as primary and some don't
I believe CCBill is still using Moneris but its probably best to double check with ccbill
