looking for a good form script

im looking for a good form script, php or cgi. it just needs a few fields for ppl to enter their email, name and phone number. also needs a function where if someone leaves a certain field blank, it won't accept it unless the person fills it in. and a flooding function, if ppl try to fill it out 1000 times over. i've heard you can use htaccess for that tho? any suggestions? free or cheap prefered.
