make clickable links from text file ??

i have a large text file full of links is there anything out there that will go through and add the herf tags so they become clickable?? Can't find anything like this..

use a program to replace "http://" with

<#a href="http://

and replace .html/.htm/.php with .html/.htm/.php>CLICK ME<#/a> to the end.

(take the #'s out, duh)
