Those adspots are the premium ad-words which cost a minimum of $5000/mn and a 3 month term committment. The ads are on a per impression basis but I don't believe Google is taking any new clients as they phase premium adwords into a PPC model as their regular adwords program is. Besides that, going up to bid against BN and Half will take a large bank account.

You have two options, become a premium adwords customer which is the $15k option and Google isn't taking new ones to my knowledge or use the regular adwords and get a really high CTR and bid high. If you have a high CTR, Google will bump you from the right column to the premium spots but they don't release what the CTR % or bid rate required for this.
this is google premium sponsorship and it is not available anymore. Last contracts will expire within 2003 and everything will follow the adwords model which is patent penting by google.
