Which first? Front-end or Back-end?

When you are making a new site do you get a designer to make the design/layout then find the programmer and do the backend. or do you get a programmer to program the entire site, then get a designer to make a layout after?

Yeah I'm just curious which way most people do it. Being a programmer, I like having the design/layout already done so I can just plug my code into it and not have to create dummy pages/menus/whatever.
