                  oh man I feel for you and seriously hope you aren't experienceing the shoulder issues I am. I was recently diagnosed with a long thoracic nerve injury.

                  The short story of it: the nerve that controls my shoulder blade is malfunctioning and causing my shoulder blade to pop out which in turn causes my shoulder to rotate forward and my shoulder to dislocate. Needless to say it can be extremely painful. It burns almost constantly and when I am not paying attention to making sure it is "tight" it will drop out of socket. There is no physical therapy that will improve it and the only surgical option is to fuse my shoulderblade to my ribcage. NOT

                  So, I have to wait a year and a half for the nerve to hopefully regenerate. I can still use my arm but I have to be sure to pay attention to keep my shoulder in place and not to raise my arm above my head or the trouble starts.

                  They say it is a tennis players injury and is caused with a repeated swinging motion of the arm so I suppose just about anyone that has that motion could very easily get it. My orthopedic doc said in his 15 years of practicing he's only seen it one other time and that was from a guy that was skydiving and got his arm tangled in the cords and had his arm jerked really hard.

                      Maybe you pinched a nerve? I have permanent nerve damage in my shoulder so I wake up with that feeling frequently. (The other muscles around compensating, pinching other nerves etc.) They have those pads that are self heating for muscle aches... (They heat up as soon as they hit oxygen) therma "something".... I can't remember the full name. They are better than a heating pad because they stick to you and keep working no matter what you are doing and you aren't limited to the plug in your wall like heating pads. I use those in combination with Aspirin and it helps sometimes.
