Something that my self taught mixed skills can installed and set up with as little pain as possible..

Or that I can have Mojohost make happen...

There was a nice form script @ http://www.tomsnewbiebooster.com but just went there and the domain is parked. Maybe you can download it via http://www.archive.org

It was located on the "site map" section and the script is called "porn mail".

You want the free version of FormMail from TecTite. Use the Easy Wizard Setup to configure it for your site, upload it and use whatever form you create.
