You have to email them and ask about it.
If the form got you no response maybe make a standard affil account then email them in there. If I remember correctly they have an email addy in there.
It takes them like two weeks to build your cam site, they will ask you to send them a logo and you can ask them to customize it once its finishes (what type of girls on front page etc).
Im at work and don't have a direct email addy on me sorry. Its something simple like support@ or something along those lines.
I haven't pushed mine yet, I guess i should.
If you go to my sm wl foreignwebcams.com, you see they changed the colors etc to match my logo. Check out camgirls site camgirls.com as another example. I forgot jacksparrows cam link but he has a black one if i remember correctly. I suppose if you want a different color scheme than your logo, they can hook you up as well, it looks like simple css changes and maybe some minor graphic tweaks.
Webcamwiz from pussycash is nice as well. You can manage some parts of it using their tools and if you want to be more complex with it, I hear you can request to use their api and set it up on your own layout. I don't have the resources to justify getting a high risk merchant acct yet or I would have done that. They have ref managers that will help you set it up etc. If you are like me and enjoy total control over your site, that may prove to be the best way to go.
