Probably the same legal doctrine as a "Terry Stop" with extended powers under the PATRIOT Act ...

you can say no and they can only search you if they arrest you, they can only arrest you if they have probable cause.
at mass transit / infrastructure locations I would probably agree to the a search.
if it was out if the blue I probably would say no thanks and have done so before in Europe.
I never yet had such a request in the United states outside an airport.
