you can say no and they can only search you if they arrest you, they can only arrest you if they have probable cause.
at mass transit / infrastructure locations I would probably agree to the a search.
if it was out if the blue I probably would say no thanks and have done so before in Europe.
I never yet had such a request in the United states outside an airport.
