If you are using wordpress http://akismet.com/ is brilliant. for the spam issues
For fake profile issue, use a email click to activate link. and maybe a recapture image on sign up as well.
I went back and forth on the 'click to activate' thing and my reasoning was if they can't be bothered to even click a link to activate their account , how active will they actually be. -
