Am I the only one waiting for SKNANB to the form?
 

I emailed them on wednesday morning with my name and last four digits on the card but still waiting for a reply.

I called the bank yesterday and they asked me to wait for an email response.

Those who already received it: How much time have you waited until they sent the form?
It seems most of you got it very quickly, at least, faster than me.

Yes indeed, for a small percentage I assumed those above 10K (which I am).

However anyone with more than 2k does not seem to be the a small portion of cardholders.

takes about 2-3 days to get it. You can also just get it from mediafire and just send it to them instead of waiting.
