I'm unable to get past the first page of this join process, but the program owner quickly got back to me and said it was working for them.

Doesn't work for me in IE, FF or Chrome. I enter in false info so I can see who they use to process and check for cross sales etc, however when I submit the page I just end up at a blank white page.

To be clear: I can get to the above URL, but if I submit the form, I get a blank white page. What happens when you try?
