I mean if this person was doing some nonprofit stuff in mainstream and became fairly popular on the web, do you think their adult affiliation could come back to bite them in the ass?

finding cures for disease and taking donations. Using forums and youtube, twitter, ect to build up my name and get more attention to my sites. Might be some celebrities involved at some point.

It most certainly is a bad idea. If all you do is adult, you need some mainstream stuff on the side.
Look at it this way: no mainstream company will ever want to hire you if you cannot show them what you did. The longer you work in adult exclusively as an affiliate only, you will have more and more problems explaining the gap in your cv.
Create some mainstream sites and maintain them.
