Didn't he have a heart attack while filming apocalypse now? The guy partied hard during his younger days.

Theres also a concept that filmmakers use called association/assimilation

put me with a group of people older than me and the audience will perceive me to be older than I am, put me in a group of people younger than me and the audience will perceive me to be younger than I am.

In 1976 Martin Sheen was already in his late 30s when filming began on Apocalypse Now. Check him out in a movie called 'Badlands' if you haven't already. That and 'Apocalypse Now' are his two best films.
