The most correct answer is "see an attorney" but I hear what you're saying. I had the advantage of knowing an adult entertainment attorney before I ever got started in this business and got a reasonably priced attorney-blessed contract but not everyone is afforded that opportunity.
Generally, a contract off the web is sufficient. Download it and tailor it to your own needs. Spend time detailing things about the use of the content. If it's for mainstream content, then I wouldn't even worry about anything else. That's me personally. Others may disagree.
Now, if it were for adult content, then I would spend more time detailing the contract and I would indeed pay an attorney to provide you a locked-tight contract. Just keep in mind that even a $5000 contract blessed by an attorney doesn't guarantee anything. Anyone can sue anyone at any time.....assuming you're in the sue-happy US of A. Just ask Donald Trump and his ex-wives.
