Small little cuts with a razor blade. Takes awhile though to do that many little shovel scoops.

Iron it out (literally) then go to a derma for a laser treatment. :2 cents:

Some of the newer inks use a PVC based pigment that when a laser hits it, instead of vaporizing, it melts into a black mess. Always fun when someone finds out they've got that kind of ink and they try to remove it. :)
