Provided the handler is not an idiot, and the safety is not off, how, exactly, does a gun "accidentally" go off?

I can understand he probably didn't intend for it to go off... so it was an "accident"... but I don't think bullets spontaneously explode.

I'm curious as to how it happened.

i didnt know there were special pilots with guns.. remember few years ago the pilots unions were against having to be armed.

Exactly, The pilot had to accidently pull the trigger. Something that never accidently happens.
