How do you organize and stay focused? (gtd/task lists)

I have a hard time organizing thoughts, tasks and other items on a regular basis. I also have a hard time prioritizing those items and completing them.

How do you keep track of the many things you have to do during the day?

I need some sort of organized system, an easy way or marking completed and leaving notes for tasks. I have tried notepads, word docs, and a few programs like rememberthemilk.com

Ive got a notebook for long projects. And I usually have a notepad open (at all times) for the most current stuff I need to work on.

I use one of those composition notebooks. I'm a big fan of writing shit down.

Downloaded evernote. Looks promising. I am going to give it a try.

Most of my To Do list is email based. Someone sends me an email and I keep it in the new section until the item is done. If it's really urgent or date oriented, I use Outlook flags to mark it.

Other than that, I've tried lists in notebooks and on the computer but I find I spend more time making and checking the lists than completing the projects so mostly, it's in my head with visual cues like URLs bookmarked and emails.
