Appears to me that they have caught a cheater and rather than ban him outright, they are monitoring the account and cheating him right back to recoup what he has already cost them.

Actually, the date in the mysql statement says 2003, but the "notes" says that the note was add in January 2007.
So this screenshot must be from some time after Jan 2007.

it seems to me like a photochop... and a careless one, since you have info that doesn't match... it says that the error is from 2003, but the notes are from 2007. Plus it shows the cams percentage at 30% which is outdated, since they now pay 35%...
and to top it off, it is hosted by JTF...
