Where did you guys learn to write English? I see this almost every day here in the forum and elsewhere. I am speaking to many of my fellow Americans. It's "could HAVE", or "should HAVE" or even "could've/should've", not "could OF" or "should OF". Thank you.

I'm betting it's that vernacular that makes people think that others are saying "of", and - perhaps in some cases - cause people to mimic it that way.
