This isn't a test, there is no hard-to-spot secret thing or right answer here. I'm just looking for what pops into your mind when you first look at the photo.

It could be anything. (and not necessarily the headless guy)

My first thought was more fucking grafitti.

Schools out and grafitti hase popped up everywhere in my neighborhood.
My friends nice speedboat got covered and they always seem to paint on something that's very hard to clean up.

Then I thought it was was " shock art " as was mentioned.

When I first observed the picture my first thought was that, assuming it is a guy holding her hand of course, that it's another one of those "he must have money" things. (assuming she's actually hot-looking from the front as well)
